  摘 要:目的 探讨乳癌(MCF-7)细胞株分泌物对凝血系统的影响. 方法 分别测定高低两种浓度的乳癌细胞分泌物对正常血浆和异常血浆的部分凝血活酶时间(APTT),凝血酶原时间(PT),凝血酶时间(TT)值和VⅢ,V因子活性的影响. 结果 两种浓度的乳癌细胞分泌物均可使正常血浆APTT(s)显著缩短(37.6±0.3)vs(31.0±0.5),(26.0±0.5),P&<0.05,而对PT无影响,使异常血浆的PT(s)延长(18.7±0.5)vs(23.6±0.5),(31.6±0.8),P&<0.05,而对APTT无影响;只有高浓度的分泌物能使TT(s)延长[正常血浆(15.8±0.6)vs(18.8±0.4),异常血浆(18.8±0.5)vs(22.4±0.5),P&<0.05].两种浓度的乳癌细胞分泌物均可使VⅢ因子活性增强(125±11)vs(170±10),(250±13),P&<0.05,只有高浓度的分泌物能使V因子活性降低(89±10vs75±9,P&<0.05). 结论 乳癌细胞分泌物对凝血系统功能有影响.

  Abstract:AIM To investigate the effects of secretions from breast cancer cell strain MCF-7on blood coagulation system.METHODS Activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT)and activities of factors VⅢand V of normal and abnormal sera were tested respectively after influenced by higher and lower concentration secretions from breast cancer.REUSLTS Higher and lower concentration secretions shortened the nor-mal serum’s APTTs [(37.6±0.3)vs(31.0±0.5),(26.0±0.5),P&<0.05],but exerted no effect on PT;the abnormal serum’s PTs influenced by these two concentrations secre-tions were prolonged [(18.7±0.5)vs(23.6±0.5),(31.6±0.8),P&<0.05],but the abnormal serum’s APTTs didn’t change.The higher concentration secretion might prolong TTs [normal serum(15.8±0.6)vs(18.8±0.4),abnormal serum(18.8±0.5)vs(22.4±0.5),P&<0.05].Secretions of higher and lower concentrations both enhanced the activity of VⅢ[(125±11)vs(170±10),(250±13),P&<0.05],but the activity of V was decreased only by the higher con-centration secretion [(89±10)vs(75±9),P&<0.05].CON┐CLUSION The secretion from breast cancer cell strain MCF-7may influence the blood coagulation system.

  恶性肿瘤患者常常因出凝血功能异常而导致病理性高凝状态(hypercoagulable state,HCS),HCS与肿瘤的病情和预后有密切关系[1,2] ,HCS可改变肿瘤细胞的表型结构和活性,使肿瘤在局部增殖、浸润和转移[3-6] .作者测定了乳癌细胞(MCF-7)分泌物对凝血系统的影响,现报道如下.
  
  1 材料和方法
  
  1.1 材料 正常血浆为中国医学科学院输血研究所正常参比血浆,n=11.异常血浆为收集临床凝血时间延长的混合血浆,n=8.部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)、VⅢ因子、V因子检测试剂盒均为中国医学科学院输血研究所销售产品.乳癌细胞分泌物为乳癌细胞株(MCF-7)72h的培养液.
  
  1.2 方法 取乳癌细胞株(MCF-7)经RPMI1640培养液72h,3000r・min-1 离心20min,取上清液,置透析袋内用聚乙二醇浓缩.以生理盐水调成蛋白5g・L-1 (浓),和1.25g・L-1 (稀)两种质量浓度.待测血浆100μL加乳癌细胞分泌物10μL(对照加10μL培养液),混匀,室温放置5min,后按试剂盒说明测定APTT,PT,TT时间.方法经典一期法.1/10稀释待测血浆100μL加缺V因子血浆100μL,PT试剂100μL,乳癌细胞分泌物10μL(对照加10μL培养液),混匀,室温放置5min,后按试剂盒说明测定V因子促凝活性.1/10稀释待测血浆100μL加缺VⅢ因子血浆100μL,APTT试剂100μL,乳癌细胞分泌物10μL(对照加10μL培养液),混匀,室温放置5min,后按试剂盒说明测定VⅢ因子促凝活性.
  
  2 结果
  
  2.1 乳癌细胞分泌物对凝血系统的影响 加入浓分泌物(蛋白质量浓度5g・L-1 乳癌细胞培养液)可使正常血浆的APTT显著缩短(P&<0.01),异常血浆的APTT无变化;使异常血浆的PT显著延长(P&<0.01),正常血浆PT无变化;使正常和异常血浆TT均明显延长(P&<0.05,Tab1).加入稀分泌物(蛋白质量浓度1.25g・L-1 乳癌细胞培养液)也使正常血浆的APTT缩短(P&<0.05),异常血浆APTT无变化;使异常血浆PT延长(P&<0.05,Tab1),正常血浆PT无变化;而对TT正常、异常均无明显改变(P&>0.05).

  2.2乳癌细胞分泌物对VⅢ,V因子促凝活性的影响 加入浓分泌物(蛋白质量浓度5g・L-1 乳癌细胞培养液)可使VⅢ因子促凝活性显著提高(P&<0.01),V因子促凝活性明显下降(P&<0.05,Tab1).加入稀分泌物(蛋白质量浓度1.25g・L-1 乳癌细胞培养液),使VⅢ因子促凝活性明显提高(P&<0.05,Tab1),而V因子促凝活性无明显改变(P&>0.05).
  
  3 讨论
  
  恶性肿瘤可分泌多种物质,经多种途径影响凝血功能[7-10] ,其中组织因子(Ⅲ)、X因子活化剂等,直接影响患者的凝血功能,使成HCS.这种不正常的高凝状态又可促进肿瘤细胞增殖、浸润和转移.鉴于不同种类恶性肿瘤影响凝血系统的机理不同[10-15] ,作者以乳癌MCF-7细胞株培养液直接加入一些血浆凝血功能实验,观察乳癌分泌物对凝血功能的影响.结果发现乳癌细胞分泌物使正常血浆APTT缩短、TT延长、VⅢ因子活性增高、V因子活性降低,使异常血浆PT,TT延长.我们知道APTT实验为内源性凝血的筛选试验,影响因素与VⅢ,IX,XI,XⅡ等有关,在以上实验中缩短原因是由VⅢ因子活性增高所致.

  本实验显示乳癌细胞分泌物通过促进VⅢ因子活性,使正常血浆内源性凝血功能亢进,从而缩短由内源性凝血系统启动的凝血过程.对异常血浆,乳癌细胞分泌物虽亦促进VⅢ因子活性,但由于凝血因子数量的减少,其总体活性不能达到一定强度,因此没有使内源凝血发生明显变化;PT实验为外源性凝血系统的筛选试验,影响因素与Ⅱ,V,VⅡ,X等有关.本实验结果显示乳癌细胞分泌物对正常血浆的外源性凝血无明显的影响,而凝血功能不良时,可通过影响外源性凝血系统,抑制V因子活性,使凝血功能更差,以至出血;本实验还显示乳癌细胞分泌物使TT延长,TT延长因素见于肝素或肝素抗凝物质增多,低(无)纤维蛋白原,FDP增多抑制外源性凝血所致.此实验表明为肿瘤细胞分泌溶解纤维蛋白原和抗凝物质所致.这些变化为乳癌分泌物使得在凝血过程中,内源性凝血系统被加强,易使患者血栓前状态或血栓形成可能性加大,血栓形成可发展成DIC.这是促凝的一面.而另一面,乳癌分泌物含有干扰外源性凝血的物质,它使V因子活性下降,凝血酶TT凝结时间延长,形成出血倾向,引起肿瘤出血[10-15] ,另外激活纤溶系统也是其中原因之一[2,16] .
