  摘 要:目的 探讨不同子宫内膜中热休克蛋白(HSP)70和90表达与ER表达的关系. 方法 用免疫组化Envision法和图像分析仪检测了30例正常子宫内膜、30例增生过长子宫内膜和53例子宫内膜癌中HSP70、HSP90和雌激素受体(ER)的表达. 结果 增生期内膜中HSP90和ER的表达(62.1±6.0)%和(67.80±4.2)%,明显强于分泌期内膜(42.0±5.2)%和(40.2±7.8)%,P&<0.05,增生过长内膜中表达进一步增强(74.2±2.2)%和(72.6±2.8)%,P&<0.05,而内膜癌中表达较增生期和增生过长内膜减弱(51.3±2.6)%和(48.7±5.2)%,P&<0.05;分泌期内膜中HSP70的表达(65.2±1.0)%,较增生期内膜增强(44.1±3.2)%,P&<0.05,内膜癌中表达较分泌期内膜更强(75.3±3.8%),P&<0.05.内膜癌中HSP90表达随病理分级升高而减弱(67.1±5.3)%,(49.1±1.2)%和(29.3±3.1)%,P&<0.05,非子宫内膜样癌中HSP90表达(51.2±3.2)%较内膜样癌减弱(67.2±6.0)%,P&<0.05;内膜癌中HSP70表达随病理分级升高而增强(48.2±5.0)%,(62.7±4.3)%和(75.2±2.8)%,P&<0.05,非子宫内膜样癌中HSP70表达(74.1±4.2)%较子宫内膜样癌增强(60.6±3.8)%,P&<0.05.子宫内膜中HSP90表达与ER表达成正相关(r=0.5715,tr =7.3369,P&<0.01);HSP70表达与ER表达成负相关(r=-0.4132,tr =-4.7805,P&<0.01). 结论 HSP90可能促进子宫内膜增生,HSP70与子宫内膜的增生、分泌均有关,主要与子宫内膜的分泌有关.内膜癌中HSP70、90表达对癌细胞的生长、分化有重要作用,HSP70对癌细胞的调亡有一定的保护作用.

  Abstract:AIM To investigate the expression of heat shock protein70,90in different endometrium and the correlation between HSP70,90and ER status.METHODS Thirty sam-ples of normal endometrium,30samples of endometrial hy-perplasia and53samples of endometrial carcinoma taken from Xijing Hospital were examined for HSP70,90and ER expres-sion by using Envision immunohistochemical analysis and computerized imaging analysis system.RESULTS Expres-sion of HSP90and ER in proliferative phase(62.1±6.0)%,and(67.8±4.2)%is stronger than in secretory phase of normal endometrium(42.0±5.3)%and(40.2±7.8)%,P&<0.05,and became strongest in endometrail hyperplasia(74.2±2.2)%and(72.6±2.8)%,P&<0.05,but decreased clearly in endometrial carcinoma(51.3±2.6)%and(48.6±5.2)%,P&<0.05.Expression of HSP70in secretory phase(65.2±1.3)%is stronger than in proliferative phase(44.1±3.2)%,P&<0.05,and strongly increased in endome-trial carcinoma(75.3±3.8)%,P&<0.05.Expression of HSP90was decreased with the loss of differentiation(67.1±5.3)%,(49.1±1.2)%and(29.3±3.1)%,P&<0.05,and HSP70was increased with that however(48.2±5.0)%,(62.7±4.3)%and(75.2±2.8)%,P&<0.05.Expression of HSP90in nonendometrioid carcinoma(51.2±3.2)%is lower than in endometrioid carcinoma(67.2±6.0)%,P&<0.05and HSP70(74.1±4.2)%was inverse(60.6±3.8)%,P&<0.05.Expression of HSP90has direct correlation with the expression of ER in endometrium(r=0.5715,tr =7.3369,P&<0.01)and expression of HSP70has inverse correlation with that(r=-0.4132,tr =-4.7805,P&<0.01).CON┐CLUSION HSP90conrtribute to cell proliferation of en-dometrium.HSP70might has a role in the proliferative and secretory endometrium,mailnly in the secretory endometri-um.HSP90might has an important role in the growth and differentiation of endometrial carcinoma.HSP70might has a protective role in the apoptosis of cell of endometrial carcino-ma.Expression of HSP90has direct correlation with the ex-pression of ER in endometrium and HSP70has inverse corre-lation with that.

  子宫内膜增生过长和子宫内膜癌常见,但其分子病理学改变却知之甚少.热休克蛋白(HSP)是一组高度保守的伴侣蛋白,HSP70,HSP90在人类子宫内膜中均有表达,受甾体激素调节,与甾体激素受体如雌激素受体(ER)及孕激素受体(PR)表达有关.我们采用免疫组化法和图像分析法探讨不同子宫内膜中HSP70,90表达与ER表达的关系.
  
  1 材料和方法
  
  1.1 材料 1999-01/2001-12西京医院病理科子宫切除和内膜刮除正常内膜30例(增生、分泌期各15例),增生过长内膜30例(简单型20例,复杂型10例),子宫内膜样癌53例(病理分级:Ⅰ级30例,Ⅱ级14例,Ⅲ级9例;组织学类型:内膜样腺癌39例,非内膜样腺癌14例;临床分期:Ⅰ/Ⅱ期42例,Ⅲ/Ⅳ期11例),所有标本诊断均经组织病理学证实.标本蜡块,连续切片,厚5μm.兔抗人HSP70,HSP90多克隆抗体为美国Maxin Biotech Inc产品,小鼠抗人ER单克隆抗体为美国Antibody Diagnostica Inc产品,山羊抗兔Envision二抗为美国Dako公司产品.
  
  1.2 方法 石蜡切片经二甲苯脱蜡,梯度乙醇致水,磷酸盐缓冲生理盐水(PBS,pH=7.2)冲洗.微波炉0.01mol・L-1 枸橼酸钠缓冲液(pH=6.0)行抗原修复(5min×2次),自然冷却.30g・L-1 过氧化氢封闭内源性过氧化物酶,室温,30min.正常山羊血清封闭非特异性抗原,室温,10min,滤纸吸去封闭血清.兔抗人HSP70,90抗体,小鼠抗人ER抗体,37℃,120min.山羊抗兔Envision二抗,37℃,30min,DAB显色,苏木精复染、脱水、透明、树胶封片.阳性对照采用已知阳性结果的肝癌切片,阴性对照用PBS代替一抗.用Leica Q500Mc图像分析仪测阳性结果的相对染色强度,每张片子选5个高倍视野(×200),以颜色为标准作图像分割,测阳性结果的相对不透光度(%).结果用SPSS统计软件处理.
  2 结果
  
  2.1 子宫内膜HSP,ER的表达 HSPs阳性信号为上皮和基质细胞的胞质和/或胞核内出现棕黄色颗粒,ER阳性信号为上皮和/或基质细胞的胞核内出现棕黄色颗粒,阳性细胞呈局灶性或弥漫性分布(Fig1).增生期内膜中HSP90,ER表达(62.1±6.0)%和(67.80±4.2)%,明显强于分泌期内膜(42.0±5.2)%和(40.2±7.8)%,P&<0.05,HSP70表达(44.1±3.2)%较分泌期内膜(65.2±1.0)%减弱(P&<0.05).简单型增生过长和复杂型增生过长内膜中HSP70,90和ER表达无显著差异.增生过长内膜中HSP90,ER表达(74.2±2.2)%和(72.6±2.8)%较增生期内膜明显增强(P&<0.05),HSP70表达与正常内膜无显著差异.内膜癌中HSP70表达(75.3±3.8)%高于分泌期内膜和增生过长内膜(P&<0.05),HSP90,ER表达(51.3±2.6)%和(48.7±5.2)%,明显低于增生期内膜和增生过长内膜(P&<0.05,Fig2).
  2.2 子宫内膜癌中HSPs表达与临床病理学指标的关系 内膜癌中HSP70表达随病理分级升高而表达增强(48.2±5.0)%,(62.7±4.3)%和(75.2±2.8)%,P&<0.05,非内膜样癌中HSP70的表达(74.1±4.2)%较内膜样腺癌中强(60.6±3.8)%,P&<0.05;HSP90的表达随病理分级升高表达减弱(67.1±5.3)%,(49.1±1.2)%和(29.3±3.1)%,P&<0.05,非内膜样癌中HSP90表达(51.2±3.2)%较内膜样癌中减弱(67.2±6.0)%,P&<0.05;内膜样癌中HSP70、90表达与临床分期、淋巴转移和肌层浸润深度之间无显著相关性(Fig3,4).对113例子宫内膜中HSP70、90和ER表达进行线性相关分析.经统计学处理,HSP90与ER的相关系数r=0.5715,相关系数假设检验tr =7.3369,P&<0.01,表明子宫内膜中HSP90表达与ER表达成正相关.HSP70与ER的相关系数r=-0.4132,相关系数假设检验tr =-4.7805,P&<0.01,说明子宫内膜中HSP70表达与ER表达成负相关.
  
  3 讨论
  
  HSPs是一组具有重要生理功能高度保守的糖蛋白超基因家族,生理、病理及环境因素均可诱导HSPs的产生,对细胞的生存起保护作用[1-4] .研究表明,HSPs与细胞的增生、分化、瘤变及凋亡等生长过程有关[5-9] .有关子宫内膜中HSPs与ER表达关系的研究国内外报道不多.有报道指出:HSP90可促进子宫内膜上皮细胞增生,子宫内膜癌中HSP70,90表达与内膜癌组织学类型和性激素受体表达显著相关[10] .我们的研究表明,HSP70,90在增生期、分泌期子宫内膜中都有表达,增生期子宫内膜中HSP90表达强于分泌期内膜,增生过长内膜中表达进一步增强,分泌期内膜中HSP70表达强于增生期内膜.说明HSP70,90在子宫内膜的增生分泌中均起一定作用,HSP90可能促进子宫内膜增生,HSP70主要与子宫内膜的分泌活动有关.

  内膜癌中HSP70,90表达有明显变化,内膜癌中HSP90表达较正常内膜和增生过长内膜减弱,HSP70表达反而增强,说明HSP70可能对内膜癌变有促进作用,而HSP90对内膜癌变有一定的保护作用.内膜癌中HSP90随肿瘤分级升高而减弱,HSP70随肿瘤分级升高而增强,内膜样癌中HSP70表达较非内膜样癌中弱,而HSP90较非内膜样癌强.肿瘤细胞分级及组织学类型和患者的预后是密切相关的:子宫内膜癌患者5a存活率随肿瘤细胞分级升高而明显降低,腺鳞癌、透亮细胞癌和浆液性乳头状腺癌等较内膜样癌预后差[11] ,说明预后差的肿瘤中HSP70表达强,HSP90表达弱.预后差的肿瘤细胞生长速度快而凋亡少,细胞分化程度低,说明HSP70过表达对癌细胞分化和凋亡有抑制作用,HSP90作用可能相反.113例子宫内膜HSP70,90与ER表达的相关性分析表明,HSP90表达与ER表达成正相关,HSP70表达与ER表达成负相关.子宫内膜增生过长和内膜癌的发生及内分泌治疗效果与雌激素作用和ER表达有关.故研究子宫内膜中HSPs表达对探讨内膜生长规律及病变机制、改善治疗措施及治疗效果等方面将有很大的指导意义.
