扶正抑瘤饮抑瘤作用的实验分析

  【Abstract】 AIM: To evaluate the inhibiting effects of traditional Chinese medicine FuZhengYiLiuYin (ZY) on gastric cancer cell lines in vitro and on mice transplanted sarcoma 180 in vivo. METHODS: MTT was used to measure the levels of the proliferation of various differentiated gastric carcinoma cell lines cultured with ZY in different concentrations. Mice transplanted sarcoma 180 was used as a model. Mice was randomly pided into three groups as follows: control group, ELF (Etoposide Leucovorin Fivefluorouracil) group and ZY group. Mice survival rate and the change of weight were observed. RESULTS: The proliferation of MGC803 cell lines was obviously inhibited by ZY in a dose dependent manner and the inhibitory rate came to the peak at (41.1±5.4)%, when cultured with ZY at concentration of 103 mg・L-1, while the inhibitory rate of 8801 and SGC7901 cell lines didn't increase with the increasing of the concentration of ZY. The inhibitive ratio of the growth of mice transplanted sarcoma by ELF therapy was 93.1% and that by ZY groups was 91%, both of which were much higher than that in the control group. The tumor weight of ELF group (0.010±0.008) g was lower than that of ZY group (0.131±0.018) g, and there was no significant difference between them, but both of them were much lower than that of control group (1.45±0.30) g. Significant weight reducing was observed in ELF group(0.43±0.15)g while the weight of ZY group(6.80±0.54)g was similar to that of the control group(5.50±2.27)g. CONCLUSION: The inhibitory rate of 8801 and SGC7901 cell lines is not related with the concentration of ZY, while that of MGC803 cell lines increases with the concentration of ZY. ZY has the similar inhibitive ratio as that of ELF in mice transplanted sarcoma 180, but it has no effects on the weight of the mice.

  【摘要】 目的: 观察扶正抑瘤饮对人胃癌细胞株体外增殖的影响,及其对小鼠移植性肉瘤的抑制作用. 方法: 应用MTT比色法,检测不同浓度扶正抑瘤饮(简称中药ZY)对不同分化程度的3株人胃癌细胞增殖的影响. 并以小鼠移植性肉瘤180为模型,随机分空白对照组,ELF化疗方案组(ELF),扶正抑瘤饮组(ZY). 观察其对小鼠移植性肉瘤的抑制作用. 结果:中药对3株胃癌细胞均呈现一定程度的抑制作用,但其对不同细胞株敏感性有差异. 中药浓度在1×10-2~1×103 mg・L-1之间,对SGC7901, 8801细胞株增殖的抑制作用与剂量无明显相关,而对MGC803细胞株增殖的抑制作用随浓度升高而增强,当浓度为1×103 mg・L-1时,抑制率最大(41.1±5.4)%. 中药对小鼠移植性肉瘤(91.0%)与ELF组肿瘤生长抑制率(93.1%), ZY组平均瘤重(0.131±0.018) g与ELF组 (0.010±0.008) g相比,均无显著性差异 (P&>0.05).   0引言

  扶正抑瘤饮(ZY)方中如黄芪、白术等多味中药已经实验证明具有一定的抗癌或免疫促进作用. 我们在临床上应用扶正抑瘤饮治疗胃癌取得了较好效果的基础上,通过实验研究进一步证实扶正抑瘤饮的抑瘤作用.

  1材料和方法

  1.1材料

  1.1.1体外实验① 细胞株:人胃癌细胞株8801和人胃低分化粘液腺癌细胞株MGC803 (西安交通大学第一医院分子生物中心提供);人胃中分化转移性腺癌细胞株SGC7901(第四军医大学西京医院消化科提供);② 主要试剂和药物:扶正抑瘤饮液(西安交通大学药理研究所生产,含生药2 g・L-1主要成分为黄芪、白术、柴胡、三棱、莪术、汉三七、蒲公英、白花蛇舌草、仙鹤草、石上柏和炙甘草的浸提液);氟尿嘧啶(5 FU,上海海普药厂生产),用培养液稀释成125 mg・L-1(Cmax×1).

  1.1.2体内实验① 瘤种:小鼠移植性肉瘤180(第四军医大学动物实验中心提供);② ELF化疗药液含5FU,VP及CF;③足叶乙甙(Vp16, 连云港制药厂生产);甲酰四氢叶酸(CF,江苏制药厂生产);扶正抑瘤饮同体外实验; ④ 实验动物:4~8 wk龄健康雄性昆明种小鼠(西安交通大学医学院动物实验中心提供).

  1.2方法

  1.2.1体外实验

  1.2.1.1细胞培养3株胃癌细胞8801,SGC7901和MGC803用含100 mL・L-1小牛血清的RMPI 1640培养液,在37℃, 50 mL・L-1 CO2培养箱中培养,用2.5 g・L-1的胰蛋白酶消化传代.

  1.2.1.2中药对人胃癌细胞增殖的影响(MTT法)取对数生长期贴壁生长状态良好的3株人胃癌细胞8801,SGC7901及MGC803,按5×107・L-1的浓度接种于96孔无菌细胞培养板(200 μL・孔-1),放入37℃ 50 mL・L-1的CO2孵箱24 h后取出. 吸弃培养板上清后加入不同浓度中药200 μL・孔-1,浓度分别为1×10-2, 1×10-1, 1×100, 1×101, 1×102及1×103 mg・L-1. 并设培养液及无细胞对照孔. 每组每个浓度设3个复孔. 加药后,放回37℃, 50 mL・L-1 CO2的孵箱,药物与细胞共同孵育72 h后,各孔加入4 g・L-1四甲偶氮唑蓝(MTT)20 μL,孵育4 h,加入二甲基亚砜150 μL,混匀0.5 h后在全自动酶联免疫检测仪于570 nm波长测定A570 nm值. 以A570 nm值间接反映存活细胞数量. 据此可推测各浓度的中药对人胃癌细胞的抑制率. 抑制率= (A无药组-A用药组)/A无药组×100%(实验重复3次,取其均值).

  1.2.2体内实验已接种移植性肉瘤180的小鼠60只,体质量20~24 g. 治疗前分别称取各组小鼠的体质量,随机分为空白对照组、ELF组及ZY组3组,每组20只,均于接种瘤株后24 h,以各组相应的药物和剂量分别给予灌胃和腹腔注射. ① ELF组[1-2]: d 1~3和d 24~27均给予ELF 0.3 mL ,腹腔注射, 1次・d-1( 5FU: 15 mg・kg-1; VP:16.5 mg・kg-1; CF: 5 mg・kg-1),先予CF 0.1 mL, 1 h后再予5FU 0.1 mL, 2 h后再用VP16 0.1 mL. 同时d 1~27给予蒸馏水0.6 mL, 口服1次・d-1. ② ZY组: d1~27给予ZY药液0.6 mL, 口服1次・d-1; 同时d 1~3和d 24~27用注射用水0.3 mL, 腹腔注射, 1次・d-1. ③ 空白对照组; d 1~3和d 24~27均给予注射用水0.3 mL,腹腔注射, 1次・d-1,同时d 1~27给予蒸馏水0.6 mL, 口服1次・d-1. 末次给药后次日处死小鼠,取出瘤体,称取瘤质量,计算肿瘤生长抑制率,抑制率= (对照组瘤质量-实验组瘤质量)/对照组瘤质量×100%. 然后称取小鼠体质量,计算小鼠平均体质量变化量. 平均变化量= ∑(治疗前体质量-治疗后体质量)/例数.

  统计学处理:各组实验数据用x±s表示,用SPSS10.0统计软件进行u检验和ANOVA检验.

  2结果

  中药对3株胃癌细胞均呈现一定程度的抑制作用,但其对不同细胞株敏感性有差异. 中药浓度在1×10-2~103 mg・L-1之间,对SGC7901和8801细胞株增殖的抑制作用与剂量无明显相关,而对MGC803细胞株增殖的抑制作用随浓度升高而增强,当浓度为103 mg・L-1时,抑制率最大(41.1±5.4)% (Tab 1).

  ELF组肿瘤生长抑制率(93.1%)与ZY组(91.0%),ELF组平均瘤质量(0.010±0.008) g与ZY组 (0.131±0.018) g,均无显著性差异 (P&>0.05),但ELF和ZY组平均瘤质量均低于空白对照组(1.45±0.30) g(P&<0.05). ZY组小鼠体质量变化量(6.80±0.54)g与空白对照组(5.50±2.27)g相比无显著性差异 (P&>0.05),但ELF组小鼠体质量变化量(0.43±0.15)g明显小于空白对照组和ZY组(P&<0.05).

  3讨论

  扶正抑瘤饮由黄芪、白术、莪术、三七、白花蛇舌草和石上柏等组成. 本方中多味中药均经实验研究证明具有促进机体免疫功能,抗肿瘤的功效. 如方中主药黄芪具有抑制癌细胞增殖、诱导癌细胞凋亡的作用. 提高机体的抗肿瘤免疫功能. 对肺癌,肝癌,胃癌均有效[3-5] .

  本实验应用MTT法, 观察扶正抑瘤饮对人胃癌细胞生长的影响,结果发现该药对3株胃癌细胞均呈现一定程度的抑制作用. 当该药浓度在1×103 mg・L-1时,对MGC803细胞具有明显的增殖抑制作用,且对该细胞株的抑制作用随着中药浓度的升高而增强;而对其他两株细胞的抑制作用与剂量间无这种关系. 显示出扶正抑瘤饮对同一肿瘤细胞、不同分化程度细胞株的敏感性有差异,其机制值得研究. 体内实验研究显示该药对移植性肉瘤180小鼠的肿瘤生长抑制率在85%以上,且与ELF组相比无统计学差异. 显示该药有较好的抗肿瘤效果.

  应用ELF化疗方案治疗后,小鼠消化系统功能出现明显紊乱和障碍,出现纳食减少腹泻等症状,体质量明显减轻. 而用扶正抑瘤饮治疗,则无明显的消化系统功能紊乱和障碍,体质量也无明显的变化. 有人曾用白术治疗5FU所致腹泻[6],取得了满意的临床疗效. 这可能就是在无对症支持的情况下,中药能取得较好疗效的原因.
